**Forms completed during a preparation session are good for 1 year. Subsequent administration sessions do not require a preparation session to complete paperwork as long as the paperwork is still valid. If the paperwork has expired, an additional preparation session will need to be completed. You and your facilitator may choose to have an additional preparation session depending on timing or changes in your plan/situation.
